Town & Country K9 ResQ - Adoption Contract

I agree this dog is being adopted for myself and will not be sold, adopted, or given to another party. I agree that if Town & Country K9 ResQ was unable to spay/neuter the dog due to sickness or age at the time of adoption, I will assume full responsibility to have this procedure done and within the timeline as determined by Town & Country K9 ResQ This adoption contract will be void if I fail to spay/neuter this dog. I will be required to return the dog to Town & Country K9 ResQ if this term is not met I agree that if this dog is under 12 mos., I will enroll him/her in a group puppy socialization/training course taught by an accredited trainer. I understand that it is my responsibility to ensure this dog is properly socialized and is able to be the best version of himself he can be. I agree that I will be a responsible dog owner when it comes to training/socializing this dog. I will submit proof of puppy training/socialization class completion within three months of adoption and understand that failure to properly train and socialize a puppy will result in nullification of this adoption and Town & Country K9 ResQ may seize this dog. I agree this dog will not be outdoors without proper supervision. This dog will not be an “outside” dog. I agree to care for this dog in a humane manner and be a responsible animal guardian. This includes supplying adequate food, water, shelter, attention, and medical care. I give Town & Country K9 ResQ permission to contact me in the near future, within reason, to assure this dog is being properly treated and cared for. I agree to keep Town & Country K9 ResQ informed of my current home address and contact information for one year, in the event I move. I agree that if at any point I cannot keep this dog, I will return him/her to T&C K9 RESQ immediately. If I ever need to give this dog up, I agree to contact T&C K9 RESQ and surrender this dog back to their care. I agree that I will NOT surrender this dog to any other shelter or animal rescue. If this dog is given to a third party, this will nullify this adoption and Town & Country K9 ResQ will retain the right to seize this dog. The emergency number I will call to return this dog is (917) 929-4810 I agree that all statements I have made on this form are true. If it is found that any statements I have made on this form are not true then the adopted animal can be confiscated and returned to Town & Country K9 ResQ, and this adoption contract will be considered null and void under the County of Westchester in New York State contract law.
